Home Care

We understand that having the opportunity to live in your own home can have an enormous impact on your confidence and wellbeing. Our Home Care team provides a quality, flexible and friendly service, delivering exceptional care.

We support people with a variety of conditions, helping them to stay as independent as possible in their own homes.

Our Care

Our Care

Comfort and wellbeing are our most important considerations, along with the promotion of independence; we celebrate the individuality of our clients and form close bonds with them. We adapt and evolve our care, tailoring it to the individual as their needs change.

We work with people with a variety of conditions, including but not limited to:

  • Older people – we have an experienced track record of delivering care and support for older people and can provide specialist nursing, dementia and palliative care

  • People with learning disabilities – our teams can help people with learning disabilities acquire the skills they need to live a more independent and fulfilling life

  • Children and families – we provide support and assistance for families with disabled children over the age of 18, or to parents who themselves have a disability

  • People with physical disabilities – our care and support teams use creative approaches to help clients with physical disabilities overcome the challenges they may face with daily tasks

  • People with mental health conditions – our teams can provide treatment, care and support for people living with a range of challenging, complex or long-term mental health conditions

  • People with acquired brain injury – we help people who have suffered a brain injury as they work along the pathways to rehabilitation

We are dedicated to providing a caring and happy experience, ensuring that the best care is given to all of our individuals, along with the best possible support for their families and friends.

Our Service

Our Service

We want our clients to feel safe, comfortable and cared for in their own homes.

Our aim is to always help our clients to live more independent and fulfilling lives, no matter what illness or disease is affecting them. To do this, we provide bespoke care packages for our clients, tailoring them so that they look after not just the individual’s needs, but also support their loved ones as well.

For us, it is not just about delivering a service. We make sure that we create a warm and supportive environment for our clients and we do our best to encourage their independence and confidence. More and more people are choosing to live independently at home for as long as possible and we are here to ensure that happens. We understand the importance of being in your own space.

Our Support

When we meet new clients in the community, this is what we do:

  • Meet the individual in their home and carefully assess their situation, identifying their needs and completing a full My Life Plan care assessment
  • Provide practical advice and possible solutions regarding all aspects of the individual’s care
  • Assist individuals or their family on availability for claiming all the financial benefits, allowances and support that they are entitled by working with local resources.

  • Help the individual and their family map out their care and support plan for now and the future, including everything from personal care, medication, diet and nutrition to personal activities, hobbies and more
  • Help the individual live with dignity, independently in their own home

There are many ways we can support our clients, including but not limited to:

  • Help around the home, from cleaning and laundry to shopping and preparing meals
  • Personal support such as dressing, bathing and help with prescribed medication
  • Companionship, being there to help with activities clients enjoy, such as days out, reading, or just having a chat
  • Sitting service (if the individual is usually cared for by their family, we can step in when they are unavailable)
  • Assistance with medication
  • Specialist advice for families
  • Dementia and palliative care

We encourage relatives to be involved in the care planning process as much as possible and are committed to providing loving care for our individuals that adapts to suit their changing needs and supports their independence and individuality.

We understand how important relatives and carers are and aim to also provide the best possible care and support for them as well as the individual.
